PCN update

  • Please keep a look out for an email from your Neighbourhood Coordinator asking you to complete a survey about how Navigator work is currently being done in your practice. Your feedback will help build a picture about how we can develop further support for practices and Navigators. Thank you in advance!
  • QHS will be running a Navigator training day later this year. Further information will be in the QHS Bulletin that is sent on Wednesdays. Please let your Neighbourhood Coordinator know if you need to be signed up to this mailing list.


Link Age Southwark

Link Age Southwark has been building friendships between older people and volunteers within the local community for over 25 years, to reduce the effects of social isolation and loneliness. They offer befriending and a variety of groups including exercise, bridge, reading, singing, yoga, lunch, reminiscence and more. They also support adults living with mild to moderate dementia, as well as providing volunteer transport, handyperson jobs and gardening where available.

To make all this happen, they rely on the help and support from volunteers. It is just one hour a week that is usually required with full training, reimbursed expenses and free DBS checks provided. Healthy lifestyle hub

The Single Point of Access (SPOA) Hub is a healthy lifestyle ‘Hub’ that will manage referrals from Southwark health professionals and Southwark resident self referrals. These referrals will be for a number of health improvement interventions such as exercise on referral, diabetes prevention, stop smoking services and mental health services for those with mild depression and or anxiety.  Once the referrals are received they are screened, risk assessed and then contacted by the Hub staff.

The Hub will help the client navigate their way to the most appropriate health improvement service and support them in setting their own personal goal(s), by using both brief intervention and motivational interviewing techniques.

The SPOA Hub will be for Southwark residents or those registered with a Southwark GP practice and are aged 18 years of age or above AND meeting at least one of the following criteria:

  • Has a BMI ≥ 30 (or ≥ 27.5 if Indian, Pakistani, Bangladeshi, other Asian or Chinese)
  • Is pre diabetic (HbA1c 42-47 mmol/mol)
  • Has a cardiovascular risk of more than 10%
  • Identified with mild anxiety and or depression.
  • Is considered physically inactive or moderately physically inactive
  • A smoker

National fitness day

National Fitness Day, Wednesday 25 September 2019, is a chance to highlight the role physical activity plays across the UK, helping us raise awareness of its importance in helping us lead healthier and active lifestyles. Physical inactivity has been listed as a leading cause of preventable diseases and conditions such as Type-2 diabetes, some cancers and osteoporosis.

National Fitness Day sees a huge range of activity options enjoyed by participants of all ages, such as ‘plank offs’, yoga and pilates classes, treadmill challenges, high-street HIIT classes, dance-offs, mass walks and many more.

Time & Talents

Spend time outdoors at Time & Talents! Join a FREE, fun and creative gardening session for little nature lovers (ages 1-5) and their parents at the Little Bugs Nature Club! Come along and get mucky in the garden! Every Friday from 12pm – 1:30pm! The flyer is available here.

If you’re a builder, electrician, carpenter, or tinkerer and you are over 55, come along and get involved. Time & Talents can help you put your expertise to good use on practical projects.
