Care navigator update

  • Feedback needed: We are looking to understand how Navigator work is currently being done in practices across north Southwark. Please could you complete the this survey to give us your feedback. This will help us to build a picture about how we can develop further support for practices and Navigators. Thank you for your help!
  • QHS will be running a Navigator training day in November. Further information will be in the QHS Bulletin that is sent on Wednesdays. Please let your Neighbourhood Coordinator know if you need to be signed up to this mailing list.


Healthy Lifestyle Hub

The Healthy Lifestyle Hub will manage referrals from Southwark health professionals and Southwark resident self referrals. These referrals will be for a number of health improvement interventions such as exercise on referral, diabetes prevention, stop smoking services and mental health services for those with mild depression and or anxiety.  Once the referrals are received they are screened, risk assessed and then contacted by the Hub staff.

The referral form to the Healthy Lifestyle Hub can be found here. You can upload this on to your EMIS system so that it auto-populates for each patient. Once uploaded onto your EMIS system, details of how to use it can be found here. Referral forms should be sent to

The Hub will help the client navigate their way to the most appropriate health improvement service and support them in setting their own personal goal(s), by using both brief intervention and motivational interviewing techniques.

The  Hub will be for Southwark residents or those registered with a Southwark GP practice and are aged 18 years of age or above AND meet at least one of the following criteria:

  • Has a BMI ≥ 30 (or ≥ 27.5 if Indian, Pakistani, Bangladeshi, other Asian or Chinese)
  • Is pre-diabetic (HbA1c 42-47 mmol/mol)
  • Has a cardiovascular risk of more than 10%
  • Identified with mild anxiety and or depression.
  • Is considered physically inactive or moderately physically inactive
  • A smoker

South London Cares – black history month treasure hunt

Moorlands Community Centre, 50a Corry Drive, SW9 8QT

On Saturday 5 October 2019, 11.30 – 1.30pm, you can learn about the influential men and women of black history through a fun filled treasure hunt! The hunt will be taking place exclusively within the community centre, all levels of mobility are catered for. So, if you fancy yourself to be a riddler or want to be inspired by the forgotten figures of this world then this is the club for you. Call them on 0207 118 0404 to let them know if you would like to attend.


South London Cares – Bermondsey Film Club

On Sunday 27 October 2019, 2-5pm, head on over to our Bermondsey film club to watch ‘Hidden Figures’, which follows the thrilling tale of the 1969 space race and the overlooked members of the NASA team that made it all possible. Please call them on 0207 118 0404 to let them know you’re attending.

Self-Management UK

Please see the leaflets for the 6-week self-management courses for mental well-being, general health and well-being and carers. These courses are face-to-face group meetings with peer tutors to help people to balance their life with health conditions by creating achievable action plans.

QHS currently sends letters and texts to practice’s patients that would be eligible for Self-Management UK courses. Please contact your Neighbourhood Coordinator if you need further information on this. Patients can self-refer by booking a place or adding themselves to the waiting list for the next course. Each course has a resource base (i.e information about local services & support). The courses also enable participants with the following skills and techniques:

  • Goal Setting
  • Healthy Eating
  • Relaxation
  • Problem solving
  • Mindfulness

The Health and Wellbeing Course (Self Management for Life Patients) is for adults living with conditions such as diabetes, asthma, COPD, persistent Pain, anxiety, and many more. For further details please visit the Self-Management UK website.
